Who we are and what this policy covers

Health Hub is operated by Rebel Solutions Inc DBA LongevityCareHub.com. This policy explains how we handle information on this website, including the forms on it and the provider listings in our directory.

Information you give us

Each form asks only for the details listed below.

Claim form

If you use the form to claim a business profile, we ask for your website, email address and phone number. We also receive the profile you chose and the page you were on. A claim reaches us as an email.

Request-a-listing form

If you ask us to add a business, we ask for your name, the business name, your email address, a US phone number, the business website and its Google Business Profile link. We also record your statement that you are authorized to act for the business. Your request is stored in our database and sent to our staff by email. The form software does not store your IP address. It uses a scrambled version for a short time to limit repeated submissions.

Email

If you write to us, we receive your email address and whatever you put in the message. Please don't include medical details.

Accounts and reviews

Health Hub has no accounts, logins or reviews.

Information collected automatically

When you visit, our hosting provider records technical details of each request, such as your IP address, browser type and the page address, which can include any search words in it. We use these records to run and protect the site, and we can view them for 3 days.

Search words you type into the directory, such as a treatment and a ZIP code, travel in the page address. The site's code does not save them. The hosting records described above can include them. Google Analytics also receives the search words in the page address.

Information about providers we list

Our directory lists businesses. For each one we publish its name, address, phone number, website, services and hours. Where a business publishes the names of its clinicians, we may list those names too.

This information comes from public online business listings and from the businesses' own websites. We use it so people can find providers. To correct or remove a listing, see Contact / Report an Error.

How we use information

  • Claim details: only to handle your claim and to contact you about it.
  • Request-a-listing details: only to handle your request and to contact you about it.
  • Emails you send us: to reply to you.
  • Request records from our host: to run and protect the site.
  • Search words: to show you results.

Who receives information

We use companies that host the site and its database, a company that delivers email from our forms to our staff, and an email provider for our staff mailbox. These companies receive information as part of providing those services. Google receives visit data from our site through Google Analytics.

Claims and listing requests reach our staff as email, so a copy sits in our staff mailbox. If you ask us to delete your information, we delete that copy too.

We may also disclose information when the law requires it, to protect people's safety, or if the business is sold or merged.

We do not sell your information

We do not sell personal information, and we do not use it for targeted advertising.

Cookies and analytics

We use Google Analytics to count visits and see which pages people read. Google Analytics sets cookies in your browser. Google receives the address of each page you view, your browser and device details, and your approximate location. We have turned off Google's advertising features, so we do not use this data to show you ads. We do not use Google Analytics on the booking pages or the provider forms. You can block it with your browser settings or with Google's opt-out browser add-on.

Your rights

You can ask us to:

  • confirm whether we hold information about you and give you a copy;
  • correct it;
  • delete it;
  • tell you which companies received it;
  • stop using it, where you had agreed to its use.

To make a request, email info@longevitycarehub.com. We need enough detail to find your information and to confirm that the request is yours. Requests are free. We answer within 45 days.

If we decline your request

If we decline your request, you can appeal by replying to our answer. We decide an appeal within 45 days and tell you in writing. If we deny it, we tell you how to contact your state's attorney general.

How long we keep information

We keep information for as long as we need it to handle your request. You can ask us to delete it sooner, as described under Your rights.

Security

We limit who can read the information you send us and protect it with reasonable safeguards. No way of storing or sending data is completely secure.

Children

This site is for adults. It is not directed to children under 13, and we do not knowingly collect their personal information.

Messages we send

We do not send marketing emails or text messages. If you contact us, we reply by email about your message only.

Provider websites and other sites we link to have their own privacy policies. This policy does not cover them.

Changes to this policy

We post changes on this page and update the date at the top. If we want to use information you gave us for a new purpose, we ask you first.
